glsl: Support redeclaration of VS and GS gl_PerVertex output.

Fixes piglit tests:
- spec/glsl-1.50/execution/redeclare-pervertex-out-subset-gs
- spec/glsl-1.50/execution/redeclare-pervertex-subset-vs
